Atrium Health Service Agreement Overview

EXHIBIT A — SERVICES


Statement of Purpose


Loc-Doc Security will provide the necessary labor, parts and equipment needed to maintain existing conditions related to mechanical hardware found on commercial doors and frames at Atrium Health facilities.

  • Work related to an addition, change of scope or requiring special system access is excluded from this agreement.

  • Key issuance will remain the responsibility of Atrium Health’s locksmith shop and technicians.

  • All electronic repairs related directly to an access control system will be the responsibility of the security vendor.

  • Diagnostics of electro-mechanical parts and repairs related to access control systems are not included in this agreement.



Response time:


  • All service requests will originate from Atrium Health personnel.

  • Loc-Doc Security will make initial contact with designated on-site personnel within (8) business hours of receiving request for service.

  • Prioritization: Atrium Health will designate all incoming requests as follows:

  • Priority 1 - Repairs that are an immediate emergency or security issue. Loc-Doc Security-Doc will respond with same day or after hours service to complete repairs.

  • Priority 2 - Essential repairs that are not an immediate security issue. Loc-Doc Security-Doc will respond within 48 hours to complete repairs.

  • Priority 3 - Repairs that can be worked into existing extended schedule. Loc-Doc Security-Doc will schedule these repairs around current workload and existing Priority 1 & 2 work orders.


Materials:


Loc-Doc Security will make the best effort to keep adequate stock of materials commonly needed to complete repairs at Atrium Health facilities. Hardware standards to be determined and set forth by Atrium Health.
