▪️It is important to us to cover each employee's insurance premiums at 100% so in an effort to continue offering this benefit we will be electing a base plan with a higher deductible beginning September 01, 2021. The in-network deductible for the base plan will be $5,000 for Individuals and $10,000 for a Family. However, with the Dual Option Plan, you may choose to pay a monthly premium and elect a Buy Up plan with a $3,500 Individual in-network deductible, and a $7,000 Family deductible.

• For the Base Plan with a $5,000 Individual Deductible and $10,000 Family Deductible, employee premiums are paid 100% by Loc-Doc Security. You have the option to add a spouse, child(ren), or family, with premiums paid 100% by the employee through payroll deduction. The monthly premiums for the Base Plan are currently:

Spouse $455.46 per month Child(ren) $367.11 per month Family $984.60 per month

• For the Buy Up Plan, with a $3,500 Individual Deductible and $7,000 Family Deductible, the cost difference from the base plan is paid by the employee. You have the option to add a spouse, child, or family, with premiums paid 100% by the employee through payroll deduction. The monthly premiums for the Buy Up Plan are currently:

Spouse $628.79 per month Child(ren) $521.86 per month Family $1,269.16 per month

• Plan renewal date is September 1 annually.
  

• There are no copays or coinsurance with the Blue Options plans. Typically, you will pay nothing the day of your visit at an in-network provider. The provider will bill BlueCross BlueShield of NC. The bill will be discounted based on the contracted rate and you will be sent the balance owed. That will be your responsibility and will go towards your deductible until it is met. You can pay for all qualified medical expenses with your Health Savings Account (HSA), including the qualified medical expenses of immediate family members, even if they’re not on your plan.

• Along with your insurance plan with BlueCross BlueShield of NC you have a Health Savings Account (HSA). The Bank that houses your HSA funds is Health Equity. You have the option to setup a personal online account at healthequity.com. Through this portal you can manage your Health Savings Account funds, check your available balance, and reimburse yourself if needed. You will also be sent an HSA debit card from Health Equity that you can use to pay for qualified medical expenses.
