Employee + 1 $52.20

Employee + Children $95.13

Employee + Family $150.88

Dental insurance is available to every full time employee and the employee coverage is paid 100% by Loc-Doc. You have the option to add dependents to your dental policy and pay for those premiums through payroll deduction.

• $50 Individual Calendar Year Deductible, $150 for a Family

• Preventative Services Covered at 100%

• 3 Routine Examinations & Cleanings Per Year

• Basic Services Covered at 80% after Deductible

• Major Services Covered at 50% after Deductible

• UNLIMITED Calendar Year Maximum (excluding orthodontia)

• Orthodontia Services Covers Children through 18 & Pays 50% of Covered Services

• $2000 Lifetime Orthodontia Maximum

Changes to your policy can be made during open enrollment unless there is a qualifying life event (i.e. marriage, divorce, birth, adoption, death, or loss of coverage).
