Vehicle Accident Process
Description: what to do if my company vehicle gets into an accident? Please follow the outlined step below.
Make sure everyone is okay and call Police to fill out a report (please do step 2 while you wait on police)
Gather information for insurance and email all info below to nathan@locdoc.net
Take Pictures of both Loc Doc and all vehicles involved in accident
Take picture of other person's vehicle plate #, telephone number
Take picture of other person’s insurance card and license
Take a picture of your own personal license
Police report number (please take picture if possible)
A brief description of what happened
Email this information to nathan@locdoc.net it will be needed to send to the insurance company.
Contact Insurance company to file claim - Philadelphia Indemnity Inc
855-490-2528 (they will request a description of what happened and a lot of the info listed above)
After talking with insurance please be ready to write down the claim number they will provide.
Contact your direct supervisor.
Complete drug test at: email stewart@totalscreeningsolutions.com to set up drug test apt. He will let you know where tests can be administered based on zip code. Send results to nathan@locdoc.net
